7 Medications That Are Sometimes Linked to Dementia — What the Evidence Really Says
The headline “7 medications that cause severe dementia and profound brain damage” is misleading. There are medications associated with cognitive problems or a higher observed risk of dementia, but that does not mean they inevitably cause dementia or permanent brain damage. Much of the evidence comes from observational studies, where the underlying illness can also influence dementia risk.
1. Anticholinergic medications
This is the group with the clearest concern. Long-term exposure to medications with strong anticholinergic effects has been associated with increased dementia risk, particularly in older adults. These drugs are used for conditions including overactive bladder, allergies, depression and some gastrointestinal problems.
2. Benzodiazepines
Drugs such as diazepam, lorazepam and alprazolam can cause short-term memory problems, confusion and impaired coordination, especially in older adults. Some observational studies have found an association with dementia, but newer research remains inconsistent and does not establish that benzodiazepines cause dementia.
3. Z-drugs
Sleep medicines such as zolpidem have also been associated with dementia in some observational research. The certainty of this evidence is low, and the relationship may be influenced by the sleep problems for which these drugs are prescribed.
4. Some antidepressants
Some studies have reported an association between antidepressant use and dementia, but the evidence is inconsistent. Depression itself can be associated with later cognitive decline, making it difficult to determine whether the medication is responsible.
5. Antipsychotics
Antipsychotics can cause significant side effects, including sedation and cognitive impairment, particularly in older adults. However, current evidence does not establish that antipsychotics generally cause dementia. Their use in older people with dementia also carries important safety concerns that should be discussed with a clinician.
6. Androgen-deprivation therapy
Hormonal treatments that reduce androgen activity, used for some prostate cancers, have been associated with dementia in observational studies. However, the evidence is not sufficient to say these medicines directly cause dementia.
7. Other medications with anticholinergic effects
This is worth emphasizing because anticholinergic effects can be hidden in many different medications. Taking several drugs with these properties can increase the overall “anticholinergic burden,” potentially increasing cognitive problems in susceptible older adults
