7 medications that cause severe dementia and profound brain damage

7 medications that can affect cognition—especially in older adults

The claim that these medicines “cause severe dementia and profound brain damage” is misleading. Some medications are associated with confusion, delirium, or increased long-term dementia risk, particularly with prolonged or cumulative exposure, but that does not mean they inevitably cause dementia or permanent brain damage.

Seven important examples are:

Diphenhydramine (Benadryl and some nighttime cold/allergy products)
Doxylamine (some OTC sleep aids)
Amitriptyline — an older antidepressant
Paroxetine — an antidepressant with strong anticholinergic effects
Oxybutynin — used for overactive bladder
Benztropine — an anticholinergic used for certain movement-related problems
Certain antipsychotics, such as olanzapine or haloperidol — particularly concerning in people who already have dementia because of increased risks of stroke, cognitive decline, and mortality.

A major issue is anticholinergic burden: taking multiple medicines with anticholinergic effects can increase the risk of cognitive decline, delirium, and falls. The American Geriatrics Society recommends avoiding many strongly anticholinergic medicines in older adults when safer alternatives are available.

Don’t stop these medicines suddenly

Some of these drugs are prescribed for legitimate and important conditions. Don’t discontinue a prescription on your own. Ask your doctor or pharmacist to review your medications, particularly if an older adult has developed new confusion, memory problems, excessive sleepiness, or repeated falls.

If you give me the 7 medicines you’re concerned about (or a photo/list of the medication names), I can explain which ones have the strongest evidence for cognitive effects and which are generally safer alternatives.
