Never Use Magnesium With These Medications” — What’s Actually True?
The word “never” is misleading. Magnesium supplements can interact with certain medicines, but in many cases the solution is simply to separate the doses by several hours, not to stop magnesium altogether.
Medicines that deserve particular attention
1. Certain antibiotics 💊
Magnesium can bind to some antibiotics in the digestive tract and reduce how much medicine is absorbed. This includes doxycycline and other tetracyclines, as well as ciprofloxacin and levofloxacin. Timing matters; for example, NIH guidance recommends taking these antibiotics at least 2 hours before or 4–6 hours after a magnesium supplement.
2. Bisphosphonates for osteoporosis
Examples include alendronate (Fosamax). Magnesium can reduce absorption. NIH recommends separating magnesium from oral bisphosphonates by at least 2 hours.
3. Diuretics (“water pills”)
Drugs such as furosemide, bumetanide and hydrochlorothiazide can change how much magnesium your body loses in urine. This doesn’t necessarily mean you can’t take magnesium, but your clinician may need to monitor your magnesium level.
4. Long-term proton-pump inhibitors (PPIs)
Medicines such as omeprazole, esomeprazole and lansoprazole can, with prolonged use, contribute to low magnesium levels. In some cases magnesium supplementation is actually used to correct the deficiency.
🩺 What about kidney disease?
This is particularly important. Healthy kidneys remove excess magnesium, but impaired kidney function can make magnesium accumulate, potentially causing dangerously high levels. People with kidney disease should speak with their healthcare professional before taking magnesium supplements.
One more important distinction
Magnesium from food isn’t the same as taking a concentrated supplement. The NIH’s upper limit of 350 mg/day for adults applies to magnesium from dietary supplements and medications, not magnesium naturally present in food. Excess supplemental magnesium can cause diarrhea and abdominal cramping; extremely high amounts can cause serious heart problems
