Hip pain: what does it mean?

Hip pain can come from the hip joint itself, the surrounding muscles and tendons, or even problems in the lower back or nerves that are felt in the hip area. The cause depends on where the pain is, what it feels like, and when it occurs.

Common causes of hip pain

1. Muscle strain or overuse

  • Often happens after exercise, lifting, walking more than usual, or sudden activity changes.
  • Pain may improve with rest.

2. Arthritis (especially osteoarthritis)

  • More common with age.
  • May cause:
    • Stiffness after sitting or waking up
    • Pain with walking or climbing stairs
    • Reduced range of motion

3. Bursitis

  • Inflammation of small fluid-filled sacs that cushion the hip.
  • Often causes pain on the outside of the hip, especially when lying on that side.

4. Tendon problems

  • Irritated or injured tendons around the hip can cause aching pain, especially with movement.

5. Hip labral or joint injuries

  • May cause:
    • Deep hip pain
    • Clicking or catching sensations
    • Pain during twisting movements

6. Nerve-related pain

  • Problems in the lower back (such as sciatica) can cause pain that travels into the hip, buttock, or leg.

7. Injury or fracture

  • A fall or accident can cause hip injury.
  • Older adults may be at higher risk of hip fractures, especially with osteoporosis.

When hip pain needs medical attention

Seek medical advice promptly if you have:

  • Severe pain after a fall or injury
  • Inability to put weight on the leg
  • Sudden swelling, redness, or warmth
  • Fever with hip pain
  • A hip that looks deformed
  • Pain that is getting worse or not improving after several weeks
