How to Get Rid of Milia: Those Annoying Fat Cysts

Milia are small, hard, white or yellowish bumps that form when keratin (a skin protein) gets trapped under the surface of the skin. They’re often called “fat cysts,” but they’re not really fat, not acne, and not dangerous. Most cases are harmless and cosmetic.

What actually works to remove milia

1. Don’t squeeze them

This is the most important point. Milia are not pimples. Trying to pop them usually:

  • doesn’t work
  • can cause scarring
  • can lead to infection or darker marks

2. Gentle exfoliation (helps prevent new ones)

Milia often form when dead skin cells get trapped, so mild exfoliation can help over time:

  • Chemical exfoliants like AHAs (glycolic acid, lactic acid)
  • BHAs (salicylic acid) for oil-prone skin
  • Use 2–3 times per week, not daily at first

3. Retinoids (one of the most effective at-home options)

Topical retinoids speed up skin cell turnover and help clear trapped keratin.

  • OTC options: adapalene
  • Prescription: tretinoin (stronger)

Tretinoin is often one of the most effective treatments when used consistently, but it can cause dryness and irritation at first, so it should be introduced slowly.

4. Lightweight skincare only

Heavy creams and oily products can sometimes contribute to milia, especially around the eyes.

  • Switch to “non-comedogenic” moisturizers
  • Avoid very thick eye creams if milia form there

5. Sun protection

Daily sunscreen helps prevent skin thickening and uneven texture that can worsen milia formation.

Professional removal (fastest method)

If milia are stubborn or near the eyes, a dermatologist can remove them safely using:

  • sterile extraction (tiny needle opening + removal)
  • electrocautery or laser in some cases

This is quick and usually leaves no mark when done properly.

When to see a dermatologist

  • Milia don’t improve after a few months of skincare
  • They keep multiplying
  • They’re near the eyelids or sensitive areas
  • You’re unsure if they’re actually milia (some conditions look similar)

Bottom line

Milia don’t respond to popping or quick fixes. The most effective approach is retinoids + gentle exfoliation over time, or professional extraction for immediate results.

If you want, tell me where your milia are (eyes, cheeks, forehead, etc.), and I can suggest a more targeted routine for that area.
